Hi everyone,went to see support nurse last week, i am currently on sulfasalozine and naprosyn. She has now started me on methotrexate (4 a week) + folic acid (1 a week).As far as i know i have to continue wih my sulf and naps.Just wondered if this wasnt overdoing it a bit.She also gave me an injection which i must say has almost made me feel normal,but i aint gonna get to excited LOL Any comments on this extra medication would be appreciated,Best wishes to all John

Hi John, sorry but can't advise about mtx but I can empathise - it's horrible to feel uncertain like this,

I'm pretty sure someone on the forum will have had very similar experiences and will hopefully be along soon x

Definitely contact consultant or secretary or rheumy nurse for clarification TODAY - sorry (nagging! I know ;) but I have felt very uncertain in the past after appointments about things and it is their job to make things clear.

Hi John,

The usual treatment now for RA is to take more then one DMARD , in fact a combination of three drugs is widely used now.
The sulfa you are taking is probably not controlling your inflammation on its own, which is why the mtx has been added .The folic acid helps to counteract unwanted effects of the mtx, but don't take them both on the same day.
The injection you had was probably a steroid injection which will damp down your inflammation and make you feel better whilst giving time for the mtx to kick in.
Just be careful not to overdo it while the steroid is working, it does tend to give a sense of well being but you don't want to put too much strain on your joints.

hello John,

Glad you got sorted. The inject are brill - I love their magic.

With ref to the tablets I was on ALL of the 3 you were on when I was diagnosed. So don't worry.
However as said before I had to stop as they upset my liver.

HI John

Pleased to hear that they have got you sorted. As Doreen has already said triple therapy is very common and each medication works in its own way. Methotrexate is the gold standard treatment for RA and many, including me, have had good results from it. I can't help thinking though ...

Methotrexate is a toxic drug and you have been put on a high dose. Normally before the commencement of the treatment you should have a chest X-Ray to check for lung condition and possible latent TB, a full series of blood tests to check liver and kidney function and be given a drug monitoring book in which your regular blood test results are recorded. You keep this book, updated with results, and take it to all your appointments so the whole team know what is going on.

At first you should be on two weekly blood tests for 2/3 months and then if all is well they become monthly. It is important because the methotrexate can lead to serious problems and its efficacy and tolerance needs to be closely monitored on a regular basis. I suppose I'm a prime example; I have been on MTX for many years but just been taken off it for the second time in as many months as my neutrophil count has dropped and I'm neutropenic (unable to fight infection basically). Without the blood tests I could have become seriously ill without realising until it was too late.

I hope that your doctor has arranged all this for you but if not you really need to contact them urgently and ask them to organise things for you before you take the methotrexate. As Liz says it is their job to make sure you receive the right information and treatment. Do let us know how you go on.

Hi John thats good news
should help alot more it is well known combined therapy of 2 dmards works well,do take easy after steriod injection makes you feel great but that most likely will have been given to calm things down til mtx kicks in bit for you.
no dont think they overdoing it at all.
know for us can feel that way but RA does need b treatd agreesively to minmise joint damage and give you good mobilty

Many thanks for the replies to my query about extra meds. Some of you are so knowledgeable about RA and being a newbie i ve no doubt 1ll pick up knowledge as i go along. This is truly a great site and its good not to feel isolated.What you said is true about the feel good factor after injections, i must add i went mad and set about the garden feeling on top of the world.Having said that i`ll slow down now in view of the warnings.Thanks again folks , take care and best wishes to all.
